Please call into Killynether Practice Reception, located in Regency Medical Centre, and ask for a Killynether Practice Registration Pack. Online Forms are available from the Business Services Organisation (BSO).
Please note once you have all the paperwork completed we request you bring it in person to reception so we can verify your ID.
Please read the instructions carefully and complete the Registration Medical Form. Return the form along with the required documentation listed in the instructions. Please ask a member of our administrative team if you have any questions about the registration process.
If you have misplaced your medical card please complete the HS200 form and return it with the other documents.
Failure to provide all the required documentation may delay the registration process.
On receipt of the completed forms an administrator will book a Registration Appointment, with a Health Care Assistant. At this appointment a patients height, weight and blood pressure will be recorded. Patients will also be required to bring a urine sample to the appointment, a sample bottle will be provided with the Registration Pack.
Although you will be registered with a named GP, patients may attend any of the doctors in the practice
